CSU Ticket Sales Request Form

The CSU on-line Box Office was a success for Little Women, the Point of Grace concert, and the Youth Alive event. Therefore, we are ready to set up future events for the campus. Detailed below are the requirements for use of this site, including the fee schedule for an event. I will need one month’s notice to set up each event properly.

Contact Information
Name
EMail
Phone Number
Does the event qualify to use the on-line site? A response of "yes" to all of the questions qualifies the event.
1. Is this event sponsored by CSU?
yes no
2. Will there be at least one group paying for tickets (e.g. Students may be free, but the general public must pay)?
yes no
3. Will someone within your department be able to run the administration of the event—box office, will call, run reports for mailing tickets, revenues, ticket sales, etc.?
yes no
4. Do you expect to sell at least 25 tickets on-line for the event?
yes no

What I need from you to setup the event:
1. Type of event—concert, play, athletic, etc.
2. Name and brief description of event
3. Event Date(s)
4. Event Time(s)
5. Event Location(s)
6. Who can come? What ticket price for each group? (students are free, staff/faculty are $10, general public is $15)
7. Is it general admission or assigned seating? We do not currently have the feature to allow the purchaser to choose his/her own seating, but will in the future. Right now, the computer assigns “best available” at the time of the purchase. general admission assigned seating
8. Do you want to mail tickets to the purchaser, have them at will call, be able to use the e-mailed receipt of the purchase, or a combination of the above? will call email both
9. What are the box office hours?
10. Will call hours and location?
11. Can the purchaser pick up the tickets the day/night of the event?
12. How many tickets are available for sale on-line? Is there a maximum ticket # per person? If there is assigned seating all tickets must go through the website to ensure correct seating. Free tickets for staff/faculty/students for general admission need to be excluded from the on-line sales. If you have assigned seating and want to give out free tickets to any of these groups, you can exclude a section from the seating in the venue and make that the “free ticket” section. For example, the left section of the Chapel can be designated as “free” tickets and excluded from the on-line sales. Just let me know what section you would like to separate out. If you want to have a second option for purchase of tickets, Lifeway Bookstore for example, you can separate out a section of assigned seating or have an allotted amount of tickets for that location if it is general admission. No announcement of other options are permitted on the www.csutickets.com website. Flyers, e-mail notices or whatever publicity you are doing for this event can advertise however you want to sell the tickets, but no notice of a phone number or other means of sale can be advertised on the www.csutickets.com site.
13. Any additional information you want to include on the description page.
14. A picture of the event, group, etc. that can be used on the splash page. email graphics to: jstrubel@csuniv.edu
15. Who from your department do you want to be able to run reports?
(max 2 users—cannot be generic)

16. Who from your department do you want to be able to sell tickets?
(max 2 users—can be a generic account for multiple users)

17. To what general ledger account do you want this charged?
18. When do tickets for this event go on sale?
———————————————————————————————————
When setting the price of the ticket, keep in mind that all fees must be paid by your department. So, if you want $10 from each ticket, the ticket price should be set at $12.50 to cover per-ticket cost of the on-line site, credit card fees and admission taxes. See the schedule below for more details. This will average out to ~$10.10/ticket, but does not include cost of mailing tickets. A separate charge can be added on-site for those wishing to use this feature.
