2008-09 South Carolina Certification Form
Required for all students receiving the following South Carolina funds: LIFE, HOPE, Palmetto Fellows, or South Carolina Tuition Grant (SCTG)

Donor Recognition Grant Form
Required for students receiving the CSU Donor Recognition Grant.

2008-2009 Verification Worksheet
Required for students selected for verification. This form must be returned to CSU with a copy of the student's and parent's (or spouse's) signed 1040 tax forms. Important: Your financial aid cannot be disbursed until you complete the verification process.

Stafford Loan Entrance Counseling Online
Required for any student accepting a loan at CSU. Complete this form online at:
www.mapping-your-future.org

Professional Enhancement Grant
Full-time evening students employed full time may be eligible for a scholarship.

Financial Aid Satisfactory Progress Appeal Form
Students who fail to meet the required standards may appeal to the Office of Enrollment Services to regain their eligibility to receive financial aid.

Dependency Override Checklist
Dependent students requesting review for unusual circumstance for independency status must submit the following documentation to the CSU Financial Aid Office for consideration.

FERPA Waiver

2008-2009 FAFSA Question 31 - Drug Conviction
This worksheet must be completed since the student reported on the FAFSA that he/she had a conviction for possessing or selling illegal drugs or because he/she left Question 31 blank.

PLUS Loan Approval or Denial
This form details the PLUS Loan process.

Stafford Loan Lender Request Form
Required for all first-time Stafford Loan borrowers. You will indicate your choice of lender for your Stafford Loan. Your loan cannot be certified until the Office of Financial Aid receives this completed form.

Stafford and PLUS Loan Lenders
CSU has comprised a list of lenders who participate in the Federal Family Education Loan Program in order to assist you in choosing a lender for your federal loan. You are free to use any lender participating in the program, even if the lender is not listed. Please indicate your choice on the Stafford Loan Lender Request Form.
